OSCE for Clinical Reasoning Creative Community

The purpose of the OSCE for Clinical Reasoning Creative Community is to collectively develop solutions that promote and measure clinical reasoning using school-based OSCEs.

Objectives

This Creative Community is designed to identify and develop incremental solutions with the goal of:

  • Enhancing the development and assessment of learner clinical reasoning skills
  • Presenting patient groups without bias or stereotypes
  • Minimizing group differences in learner outcomes
  • Enabling all institutions to better support learner skill development across the continuum of medical education and training

Community Updates

October 2023 Update

Over the past 15 months, the OSCE for Clinical Reasoning Creative Community has been engaging faculty representatives from the selected 10 medical schools as our thought partners and research collaborators.

This engagement has now branched out to medical students with the recent launch of the Creative Community Student Advisory Committee, a group of 10 medical students representing a diverse cross-section of students from the Creative Community schools. Each student's unique background, identity, and experiences will bring a different perspective to the committee. With a dual role as Content Reviewers and Communications Liaisons, these students will engage in meaningful discussions and provide crucial feedback to enhance the project’s outputs.

The Advisory Committee includes the following students:

DEI Review and Framework Feedback

The committee will review the developing OSCE for Clinical Reasoning assessment through a lens focused on diversity, equity, and inclusion. Additionally, by offering unique insights into the challenges and opportunities within their educational journeys, the students will be essential in the development of a formative feedback framework for clinical reasoning skill development.

Communications Liaison

Serving as a bridge between the project and fellow students, committee members will share project updates and piloting opportunities. They will also be the voice of their peers, communicating student inquiries to NBME. We hope that this will foster a genuine connection with the student community.

By collaborating with these students, NBME is taking an intentionally inclusive approach to the development of assessments and resources that are responsive to the diverse and evolving needs of the medical community. 

 

January 2023 Update

Since the selection of our Community members in April 2022, NBME staff and Creative Community members have been engaged in the following activities:

  • Ideating solutions to assess clinical reasoning using the OSCE format: Numerous challenges were described and prioritized, and the Community has begun to consider creative solutions, varying from new post-encounter exercises to new case formats to new platforms of assessment.
  • Formulating sub-teams to focus on distinct competencies within the domain of clinical reasoning: Each team is focused on  specific clinical reasoning skills.
  • Ensuring alignment with evidence-centered approaches to assessment development: While each sub-team works independently to ideate solutions, NBME staff provide guidance on evidence-centered principles to ensure that the assessment development process supports the inferences that educators want to make about learners.

Looking ahead into 2023, the OSCE Creative Community will plan proof-of-concept pilots for the new assessment approaches developed from the sub-teams and begin to engage medical students in this work.
